‘Notice explaining USCIS action’ is either an RFE or intent to deny. Just read the letter & respond to it thoroughly with all the documents. Make sure you have a lawyer. They want to approve it but maybe not be able to because you have not provided satisfactory documents. Usually when you provide the documents that’s missing, they will approve it.

keep your fingers crossed because most times when they send you a letter like that one it's always noid. or reminder to submit a document with a deadline.
