part 1: the Form I-130 Instructions indicate that the petitions filed on behalf of a spouse must be accompanied by a completed and signed form I-130A, supplemental information for spouse beneficiary…
part 2: Although you submitted the required Form I-130A, it is incomplete because needs (beneficiary's) required information and or the beneficiary’s signature (if apply).
Submit a fully completed and signed Form I-130A, Supplemental Information for Spouse Beneficiary. If your spouse is overseas, Form I-130A must still be completed, but your spouse does not have to sign Form I-130A.
A valid signature consists of any handwritten mark or sign made by an individual, and does not include signatures created by a typewriter, word processor, stamp, auto-pen, or similar device.
** I received a Request for Evidence form stating this.. does this mean I need to fill out a new form with my husbands signature? my husband is the spouse who needs a visa. **

It was the same information I received from USCIS. I and my spouse filled it out without signing before the submission and we received a confirmation from the USCIS that they've started working on our case upon responding to the RFE.
Just download a new I-130A form, make sure to look for the latest version, and fill it out. If your filing was done online, make sure you respond to it online, and if it was done in paper filing form, then you'll have to mail it to them.

what if my husband is overseas? it says if the non US spouse is overseas they don’t have to sign it, but I’m with him overseas and we did do it online. so should we fill out a new form and he sign it and then we upload it online?
also I should add I messed up big time on the form. I filled out my information(parents, address, jobs…not my spouse.) so I think I need to make a whole new form with my husbands info(the non US spouse) and then we BOTH sign it or just him?
